Team Sign Up Form

I, the parent/guardian of the participant, acknowledge and fully understand that
basketball is a sport that involves physical contact and carries a risk of injury. I hereby
release, waive, discharge, and hold harmless the Heart of Hoops Classic Tournament, its
organizers, sponsors, staff, volunteers, and host facilities from any and all liability,
claims, or demands arising out of or related to any loss, injury, or accident that may
occur as a result of participation in this event.

I also grant permission for emergency medical treatment to be administered if
necessary. I acknowledge that photographs and/or video may be taken during the
tournament and consent to their use for promotional purposes.

By signing below, I confirm that I have read and understood this waiver and agree to its
terms.

CODE OF Conduct: For Players,
Coaches, Staff
The Heart of Hoops Classic is dedicated to maintaining a competitive, safe, and respectful
environment. By signing below, all players and coaches/staff agree to uphold the
following standards:

Players
Compete with respect, integrity, and sportsmanship at all times.

Follow all tournament rules and instructions from officials.

Respect teammates, opponents, coaches, referees, and staff.

No fighting, taunting, or use of profanity.

No use of drugs, alcohol, or tobacco at tournament sites.

Play hard, play fair, and represent your school/team with pride.

Coaches & Staff
Lead by example and promote good sportsmanship.

Treat officials, opponents, and players with respect.

Take responsibility for the behavior of your team, both on and off the court.

Abide by tournament rules and enforce them with your players.

Place player safety, development, and well-being above winning.

Communicate respectfully with referees and tournament staff at all times.

Disciplinary Actions

The Heart of Hoops Classic reserves the right to enforce consequences for violations of this
Code of Conduct, including but not limited to:

Warning or removal from the game

Ejection from the facility

Forfeiture of games

Disqualification from the tournament


My team’s spot is not guaranteed until payment is received.

Deposits are non-refundable.

Full payment must be completed by [insert deadline date] to remain
eligible.

Acknowledgment

By signing below, I acknowledge that I have read and agree to abide by the Heart of Hoops Classic Code of Conduct.

Payment has to be made by February 21st 2026 to remain eligible
